Position Summary LPN: The RN Clinic Nurse under the direction of the Clinic Nurse Supervisor, the Director of Clinic Operations and/or Population Health Nurse, will work with advanced clinical knowledge and use consistent and independent discretion, critical thinking and judgement to provide: patient education, recommendations based on current research, perform patient assessments as applicable, apply or use established protocols as needed, assess and assist with the management of chronic care patients, patients with wounds or other identified needs and communicate effectively with their PCP/Specialists.
Using the Nursing Process, the RN Clinic Nurse will also evaluate patient needs and assist with coordinating care, inform patients of community resources, and improve overall continuity of care. RN will develop an individualized plan of care with patient focused goals specific to individual needs, review care plans on a scheduled basis and revise as needed.
RN will provide education to patients and family members, perform nursing functions, and support in both Specialty and RHC Clinic as assigned. This includes delegating various tasks as appropriate or needed, assisting providers with procedures, patient assessments or result management, and assisting or providing certain education or services within RN scope of practice.
Other duties that pertain to this position include but are not limited to performing CCM services, AWV, Wound Care, and Diabetic Education. Obtaining certification of various specialty’s that pertain to Nursing.
